REQUIRED SUPPORTING DOCUMENTS

  • Letters of Recommendation (2): Each letter must be submitted by a member of faculty, preferably a supervisor or colleague, who can describe in detail the applicant’s academic, research, and leadership capabilities.  The letters should also describe the fellow’s potential as a future faculty member.  Completed letters of recommendation must include a signed cover letter on university letterhead.

  • Current Curriculum Vitae

  • Letter of Support: This letter must be submitted by either the Dean or President of the nominating institution.

  • Personal Statement: The brief statement is not to exceed 500 words and should address the   applicant’s commitment to pursuing an academic career, and how he/she hopes to impact the health of the Hispanic community.

  • Proof of Citizenship or Eligibility to work in the United States

  • Development Training Prospectus: Each applicant should design and propose a framework for an individualized study program to achieve specific research goals.

    • Institutional Commitment & Experience: The applicant institution must provide a brief description of the institution’s commitment to Hispanic students and to training the proposed fellow.  Documentation of past experience with similar programs should be provided along with how this training program will enhance the institution’s commitment to Hispanic health.

    • Program Curriculum: should include courses and/or seminars in each of the following five areas: 1) clinical epidemiology and biostatistics, 2) clinical trials and research design, 3) scientific writing (abstracts, manuscripts and grants), 4) health outcomes research, health economics, and health policy, and 5) medical ethics. 

    • Linguistic and Culturally Appropriate Competencies: A description of proposed activities and methods to be employed to enhance and/or develop the fellow’s linguistic and culturally appropriate competencies should be included.

    • Identification of Mentor: A senior faculty member (or an individual of equivalent status), with expertise in a related area of research who will serve as a mentor. A letter of commitment must be provided by the individual who would serve as the mentor.

    • Training Sites: Training must take place at member institutions.  In addition, all trainees will be required to attend the HSHPS Annual Conference.

    • Research Plan: A description of the fellow’s proposed research must be provided. The description should not exceed 10 pages and should include the following:

      • Research Problem and its significance to Hispanic health

      • Research Objective

      • Prior studies undertaken by faculty mentor(s) pertinent to the current proposal

      • Research Design

      • Methodology

      • Evaluation/Analysis Plan

      • Institutional resources for completion of project

      • Project Time Table

    •  Junior Faculty Appointment: The purpose of the HSHPS/OMH Fellowship Program is to help improve the number of Hispanic faculty at our member institutions. It is expected that the institution will make every effort to appoint the fellow to the institution's faculty. A letter of intent describing how that process would be carried out is required.

    • Program Evaluation Plan: A description of the evaluation method/tools that will be used to assess the completion of the educational objectives of the fellowship program must be included.

This plan will be finalized if the Fellow is selected as a member of the 2005-2006 Fellowship Class.  The Prospectus outlines a Fellow's specific learning goals and objectives during the Fellowship year, as well as measures for determining success. In preparing the prospectus, fellows should consider the new professional and personal skills and knowledge they will need to develop to help them achieve their goals.

  •  Documentation of any in-kind support to be received

An applicant institution may supplement the fellow’s salary at the level given to similar trainees according to institutional policies.
